ClickUp Forms: Turn Requests into Tasks
How ClickUp Forms collect requests and create a task for each submission, including questions, sharing, plan limits, and when a form is worth it.

A ClickUp Form turns each submission into a task in a List you choose. Use one for intake such as feedback, project requests, or job applications. Conditional logic is limited to Business Plus and Enterprise.
A ClickUp Form is how work enters the workspace without someone retyping an email. The person filling it out sees questions. ClickUp creates a task in the List you picked, with the answers stored on that task. That is the whole point: intake and the task list are the same system.
Prices and plan names below were checked in October 2026 against our tool data and ClickUp Help. Plan caps change, so confirm them on the ClickUp pricing page before you publish a form to clients. The ClickUp hub links the rest of the setup guides.
What is a ClickUp Form?
ClickUp Help describes Forms as a way to gather information and put it to work. You can start from a template for feedback, project intake, an order, a job application, or an IT request, or you can start from scratch.
You create a Form from the Views Bar, from Forms Hub, or from a Space, Folder, or List. A form created in the Views Bar is also a Form view on that location. Submissions land in the List you select when you create the form.
This is different from a doc that asks people to "fill in the brief." A doc is something you edit. A form is something you submit, and the submission is a task your team can assign, status, and track. If you already use ClickUp custom fields, those fields can be the questions.
What happens when someone submits a form?

ClickUp Help says forms are published by default, so check that a draft is not already live before you finish the questions. You can preview first.
File uploads through a form are capped at 25 MB per file, whether the question maps to the task's Attachments field or to a Files Custom Field. That cap is separate from files you attach directly to a task later. For a larger file, ask for a cloud storage link in the form, or upload the file on the task after it exists.
On mobile, people can submit a form or share its link. ClickUp Help says you cannot create or edit the form itself in the mobile app.
Which questions can a form ask?
Questions are task fields and Custom Fields. Renaming a Custom Field on the form changes the label people see on the form. It does not rename that field everywhere else in the Workspace.
From a question you can edit the name and description, hide the question, or require an answer. Supported types are listed in ClickUp's article on adding questions. Do not assume every Custom Field type can be a question. Check that list when you build.
Hidden fields are useful when you already know the respondent's email or a category. ClickUp Help says you can hide Email, Text, URL, Dropdown, Priority, or Number fields and pass the value through the form URL or embed code.
An Information Block can sit between questions with richer content, such as an image or a short table, so the form can explain the request without pretending that explanation is an answer.
Conditional logic is the feature people expect and then cannot find. ClickUp Help limits rules and conditions to Business Plus and Enterprise. A rule can reveal another question, and a revealed question can have its own rule, up to five levels. On the one-question-per-page layout, a revealed question gets its own page after the question that triggered it. Our price table lists Free, Unlimited at $7 per user/month billed annually, Business at $12, and custom Enterprise. It does not list a Business Plus price. If you need logic, confirm that plan on the pricing page before you promise a branching form.
How do you share a form?
Once the form is published, Copy link gives you a URL. Sharing settings include an expiry on the link, whether search engines may index it, and an embed code for a webpage. Autosize embed height is on by default.
Account Authenticated Forms, when that setting is on, restrict viewing and submitting to people who already have access to the Workspace. Use that for internal requests. Leave it off when a client or a candidate who is not in the Workspace needs to submit.
You can also share a private form with people in the Workspace. ClickUp notes that private view sharing is in early access and that future limits may apply.
After submissions exist, two analysis options are documented:
- Ask ClickUp Brain about the submission tasks in that Space, Folder, or List. Brain is a separate add-on in our data, from $9 per user/month, not part of the base Unlimited price.
- Build a dashboard from those tasks if you need a count by status, type, or owner.
ClickUp automations can share the new task with the person who submitted, without giving them the rest of the List. That is the practical "we received your request" path. It is not the same as inviting them as a member.

When a form is the wrong intake tool
Use a form when the same questions repeat and each answer should become a task: client requests, bugs, content ideas from other people, vendor asks.
Skip the form when:
- You are the only person creating the work. Add the task directly.
- The "form" is really a brief with paragraphs of context. Write a ClickUp Doc and link the task.
- Respondents need to watch a board move. A shared List or a guest view may be clearer than a one-time submit.
- You need branching questions and you are on Free or Unlimited. Either simplify the form or budget the plan that includes logic.
Start with one form and one List. A feedback form, a project intake form, and a job form in the same week is three queues nobody will watch. One queue, required fields, and an assignee on submit is enough to learn whether the team will actually work the tasks.

Each submission creates a task in the List selected when the form was created. You can open those tasks from the form's responses control or export standard fields as CSV.
